Live Well at Home Grant - SMOC

Southwestern Minnesota Opportunity Council, Inc. (SMOC) has been awarded $96,033 through DHS Aging & Adult Services Division to carry out the Live Well at Home Grant in Pipestone, Rock, Murray and Nobles Counties. This grant is for older adults and adults with disabilities to stabilize and maintain their housing to live independently in their community. Services will include providing assistance by securing needs of essential furnishings or personal items as well as home modifications to address the quality of life and safety issues. Home modifications that could be recommended, but not limited to, would be handrails, ramps, walk-in showers, high boy toilets, main floor laundry access, roof repairs, electrical issues, tripping hazards, wheel chair access and so much more. Some of the major improvements will need to be completed in collaboration with other programs and resources such as weatherization, MHFA or other local programs that would be available. Our goal is to prevent older adults, elders and adults with disabilities from moving to a nursing facility, due to quality of life and home safety issues. SMOC currently has a waiting list of 13 households who are needing assistance with home modifications and to know that we are able to help these individuals stay independent is incredible!
